For use by CNM FACULTY AND STAFF ONLY.   Use this form to report phone or data port problems or changes.The work order will be entered as soon as the Helpdesk receives it during normal business hours.

To submit a work order, copy and paste a description from the list below into the "Problem Call Description"box or enter your own description.

Voice Mail (Audix):
1.   Voice Mail box is locked. Please unlock. (I remember my password)
2.   Forgot my Voice Mail password.  Please default.
3.   Add Voice Mail to extension.
     (Your supervisor must also send an email authorizing Voice Mail to the helpdesk)
4.   Remove Voice Mail from extension.
5.   Voice Mail previously owned by another individual.  Recycle Voice Mail for new employee.
6.   Voice Mail has another person's name recorded.
7.   Need to re-record Voice Mail greeting(s).

Add/Remove/Change:
1.   Add Long Distance.
      (Your supervisor must also send an email authorizing Long Distance to the helpdesk)
2.   Add extension in room. A port for this extension already exists.
      The Position Name is:_____________.
3.   Add a port for an extension.
4.   Remove extension from room.
5.   Swap extension #'s ________, _______, etc.
6.   Move extension # ________, to Room ________. (For the same person and position)
7.   Change directory listing to #_________.  (Type in new department and /or campus if applicable)

Call processing:
1.   Put ext # (s) _______, _______, etc., in same Call Pickup (CPU) group as ext # ________.
2.   Add ext # (s) ______, _______, etc., in same Call pickup (CPU) group as ext # ________.
3.   Set ext # (s) _______, _______, etc., to cover ext # (s)_______, _______ etc.
4.   Remove ext # (s) _______, _______, etc., from Call Pickup (CPU) group with ext # _______.
5.   Remove ext # (s) _______, _______, etc., from coverage to ext # ________.

Repair / Problem:
1.   No dial tone.
2.   Extension has a dial tone but cannot dial out.  There is an intercept tone (high/low tone).
3.   Cannot dial off campus.
4.   Cannot dial long distance on this extension.  (Make sure you have requested Long Distance on this
      extension)
5.   Extension is noisy or crackles.
6.   Party can hear me, but I cannot hear them.
7.   Party cannot hear me, but I can hear them.
8.   Numbers are stacking up on the display. (Did you move the phone yourself?)
9.   Display is not showing any information.
10. Replace handset cord.  The color of the phone is ___________.
11. Replace line cord.
