CNM Employees Microsoft Software Work-at-Home Rights Software Check Out

  1. All CNM employees are eligible (sorry, this does not include student employees) to check out certain Microsoft software titles that are part of the current Microsoft Campus Agreement.
  1. Employees are required to pay a fee for each software title for the right to use that software title at home
    for work purposes. (Please refer to fees below) Employees should click on the Submit button at the bottom of this page when finished filling out the form to verify that the software titles are available and you are eligible before paying for the software at the cashiers office. Someone from the helpdesk will contact you when we receive this form.
  1. Employees will pay a fee at the Main Campus Student Services Cashiers Office (refer to account code MSDK). There are no refunds. Software titles are only available for pickup at the ‘A Building A127-N’ Helpdesk with the receipt that shows the payment for each of the software title(s). The original receipt will be attached to this form for archival purposes.
  1. The installation of each software title is allowed on one personal computer owned by the employee.
    The Work-at-Home rights software CD allows two installations per CD, the second installation may only be used for recovering from failure to the employee’s personal computer. Employees are allowed to check out only one software CD of the available software titles.
  2. ITS, CNM and Microsoft Corporation are not liable for home personal computer repair or replacement from software installation of the work at home media.
  3. The software title CDs and the software installed are useable by employees as long as they are employees of CNM. When an employee terminates employment with CNM the software must be removed from the employee’s computer or laptop.

Microsoft Campus Agreement :

All software under the MS Campus Agreement will be picked up at the CNM ITS Service Desk (helpdesk) at Main Campus (A127N). Microsoft charges for the media for the software titles CD’s; this charge is passed on to employees who wish to check out software to install on one computer at home. Employees who wish to check out software under the MS Campus Agreement are required to verify that the software is available, by clicking on Submit on this form. Employees are required to make a check out fee (for each software CD title) at the Student Services Center Cashiers Office (refer to account code MSDK). The receipt from this fee must be taken to the ITS Service Desk to verify your payment and pick up your CD(s). You will be required to sign a check out form and your receipt will be attached to the form as well. There will not be any refunds for software titles not available or checked out previously, so submit this form before you go to the cashiers office to verify availability and eligibility of software titles before making your fee payment.
